Registration Process

Please print a copy of the St. Thomas More Parish Registration Form. Blank forms are available on the bulletin board in the Gathering Space. You may also call the parish office at 847.888.1682 to request a form.

Please complete the information to the best of your knowledge. BE SURE TO PRINT CLEARLY. Mail the completed form to:

The completed form may also be placed it in the Sunday Collection Basket.

Your information will be entered into the St. Thomas More parish database, as well as, the Diocese of Rockford database. Your family will be assigned an ID Envelope number from St. Thomas More. You will also be assigned a Diocesan ID number. Approximately one month after you submit your registration, you will receive an invitation from the pastor to attend a “New Parishioner Orientation”.
